CCDS TEAM DRINK ORDER FORM
If you would like to save yourself the hassle of buying and lugging sport team drink to school for your assigned game, we are offering you the opportunity to purchase them iced and packed in a cooler through the Athletic Boosters. Simply fill out the form below and attach your payment and return it to Theresa Hirschauer, Athletic Director at CCDS.
The order form and payment must be received in the athletic office at least two days prior to your assigned game day. You may pay with cash or a check made payable to CCDS ATHLETIC BOOSTERS.
